PLA doesn't come out properly for the first 50mm or so, then everything is really nice. How can I fix this?

I set a skirt to start with as this let's the PLA come out before getting to the model.

More information. Define "doesn't come out properly". Lots of things fall into this category and the remedy will be different for each of them.
